Enrollment Requirements:

  1. Send a complete Merchant Application and Merchant Fax cover sheet.
  2. Copy of drivers license
  3. Copy of voided check in which the credit card transactions are to be deposited.
  4. Copy of last 2 credit card statements.
  5. If you currently have Discover or American Express accounts be sure to supply Merchant ID Number for each.
  6. Fax all the above information to TRIO: Fax # 1-325-437-4069.
